Livewell holds a comprehensive home care license through The Minnesota Department of Health and A PCA Agency license through MN Department of Human Services.
We accept clients on private pay, CADI Waiver, Elderly Waiver and Traumatic Brain Injury Waiver.

We can provide services in your home under our 245D license. Including, Homemaking, 24-Hour Emergency Assistance, Companionship, Personal Supports and more.
Assisted Living Facility
We own and operate an Assisted Living Residential Home in Brooklyn Center, MN. We provide care and residential services to the disabled and the elderly.
